I give permission to iHealth Fulfillment, and/or parties designated by iHealth Fulfillment to use this photo and testimonial in all forms of media, for any and all promotional purposes including advertising, audiovisual, exhibition, display, or editorial use. I further consent to the use of my name in connection with the image and testimonial by iHealth Fulfillment and/or parties designated by iHealth Fulfillment. I understand and agree that I will not receive any payment for my time or expenses related to this photo and testimonial or any royalty for the publication of the photo and testimonial or the use of my name and I hereby release iHealth Fulfillment and/or any parties designated by iHealth Fulfillment from any such claims.
In providing this consent, I understand and acknowledge that any photo and testimonial I provide which are posted on the iHealth Fulfillment website, social media accounts or any other forms of media for any and all promotional purposes may be widely circulated to the public, and may be viewed by anyone with Internet access, beyond the control of iHealth Fulfillment. If I provide consent, and then later revoke my consent, I acknowledge that although iHealth Fulfillment will remove the photo and testimonial from the above mentioned media, it may not be possible to remove all traces of personal information from the Internet. I hereby release iHealth Fulfillment and its legal representatives from all claims and liability relating to said photo and testimonial.
